Starting a New 4-H Club in Maricopa County

To start a 4-H club in Arizona, you must have at least one Certified 4-H Volunteer (DCC). Please visit the Volunteer Resources page to find out how to become a Certified 4-H Volunteer. Once you have at least one Certified 4-H Volunteer (DCC), your club may start meeting as a 4-H club. The requirements for 4-H clubs is at least 5 members from 3 families.

Please fill out this form New 4-H Club Request Form and return to the 4-H Office.

What is the next step?

The next step is to make your club official!  Clubs are required to do this to raise funds, collect dues, and use the national emblem. To do this, you need to complete the New Club Enrollment Packet and the Charter Application Packet. Once completed, please return to the 4-H Office.

Enrollment

Clubs, youth members, and volunteers (DCC) enroll annually at the beginning of the 4-H Year, or when a new club is stated.

Banking/EIN

Clubs handling funds are required to obtain a checking account. 4-H Clubs are required to obtain an EIN number from the IRS. Use this form to complete the process. Please review the Club Chartering and Financial Management Guide.

Insurance

Annual member insurance is purchased by the 4-H Office for all active members and leaders. 

Liability Insurance coverage for third party meeting locations is extended by the University of Arizona to locations where 4-H Activities are being held. This information is gathered from the Meeting Calendar and Program Plan in the New Club Enrollment Packet.
