Submitting samples to the Yuma Plant Health Clinic
Selecting samples
- If you only have one plant that can be submitted, pick the one plant or plant part that is the best example of the problems/symptoms you are concerned with.
- If this is a problem in a commercial planting, please send multiple plants with their roots attached and, when possible, at different stages of sickness (i.e. one healthy plant, one in early symptom development, and one in late symptom development).
- Please note on your form if other plant species in the collection area are also showing similar symptoms. Please feel free to submit more than one species together.
Complete the Plant Sample Submission Form
- Complete the Plant Sample Submission Form and include as much detail as you can as this information is essential for accurate diagnosis.
- Printed plant sample submission forms are available at the Yuma Agricultural Center (6425 W 8th St, Yuma, AZ 85364) on the tables outside Room 109. Feel free to take extra copies for future needs
Dropping off samples
- Complete the Plant Sample Submission Form and include it with you samples.
- Leave samples with the completed form on the table outside Room 109 at the Yuma Agricultural Center (6425 W 8th St, Yuma, AZ 85364).
- Drop samples off by 3:00 pm Monday through Thursday to allow us to process the samples the same day you drop them off. This will get you the fastest diagnostic result.
Mailing samples
- Complete the Plant Sample Submission Form and include a printed copy with your samples.
- Complete our USDA APIS PP!-526 Permit and include a printed copy with your samples. This permit form is required for all mailed samples. Contact Chris Detranaltes to request a printable copy.
- Double bag samples with zip lock bags.
- Ship samples overnight express to Yuma Plant Health Clinic 6425 W 8th St, Yuma, AZ 85364.
