Maricopa County Record Book Forms and Guidelines

Record books are an integral part of the 4-H learning-by-doing experience.  Besides being able to showcase their hard work through a record book, 4-H’ers gain valuable record keeping skills.

Record Books submitted are judged by community volunteers using the Maricopa County 4-H Record Book Scoring Rubric. Printed rubrics will be available at the Maricopa County 4-H Office to be included with each record book. Once Record Books are judged, ribbons are placed, and results are recorded, Record Books are returned to leaders. 4-H'ers with the highest Record Book scores will be recognized at the Annual Awards Ceremony. 

 

National 4-H Congress

Senior 4-H members who have submitted a Record Book for the past 3 years (2013-2016) are eligible to apply for National 4-H Congress by submitting a 2017 National 4-H Congress Youth Application Cover Sheet with their Record Book.  Top entries are chosen and sent to the Arizona 4-H Office. Arizona 4-H will then choose a group of the top entries to attend National 4-H Congress in November. 

 

Record Book Forms

Please submit Record Book forms in the following order:
(Fillable forms may be accessible online depending on the browser being used.  Forms can also be downloaded and opened with Adobe to be used a fillable pdf offline)

  1. Arizona 4-H Member Record
  2. Arizona 4-H Project Record (Primary project - this is the project that will be considered for Record Book Awards)
  3. Maricopa County 4-H Financial Summary (place behind financial form, page7, of Project Record  - see Financial Summary Definititons for help)

And if you have more projects...

  1. Addititonal Project Record(s) (USE ONLY IF YOU HAVE MULTIPLE PROJECTS, additional projects can be added from here on)

The current year should be placed in front, and should have previous years behind. Please use additional covers if record book is too large.

Record Book Covers are available for purchase at the Maricopa County 4-H Office for $3.00 each or they may be purchase online at shop4-h.org.

Record Book Trainings

Record Book Trainings are offered twice during the 4-H year (usually in October and March). Please check the calendar for the next training (dates are usually published in August).

Record Books Due Date

Record Books are due on June 5th, 2017 by 5 p.m. to the county office but all 4-H'ers are encouraged to check with their club leader for their club due date (this may be an earlier date to allow the leader to bring the entries to the county office on time).
